How do I get access to my medical records?

You can request access to your medical records by contacting your clinic directly, or through the treatment companion app.

If you need to access your medical records, contact your Care Fertility clinic or request them via the treatment companion app. If you request your records via your clinic, clinic staff will provide you with a request form and help you complete the necessary steps to obtain your records securely.
